Make a Power of Attorney
The form on this page should be used if you wish to make a Power of Attorney – which is used to appoint an Attorney.
Your Attorney has the power to make financial decisions for you, and can deal with your assets (such as accessing your bank account to pay bills), subject to any limitations referred to in the appointment and under the Powers of Attorney Act.
You can determine when the appointment of an Attorney takes effect – Immediately, during a specified period, or only after a medical practitioner certifies that your are unable to manage your own affairs.
The form below will guide you through these options. If you make a mistake or wish to make any changes after you submit the form don’t worry, as you’ll have a chance to make changes before you sign.
Looking to appoint a Guardian?
An Attorney appointed under a Power of Attorney does not have the power to make Health Decisions for you. If you wish to appoint someone to make Health Decisions for you in the event you are unable to do so for yourself then you will need to appoint an Enduring Guardian.
